As a Telephonic RN Case Manager, you’ll support a diverse member population with education, advocacy and connections to the resources they need to feel better and get well. Instead of seeing a handful of patients each day, your work may affect millions for years to come.

This is high volume, customer service environment. You’ll need to be efficient, productive and thorough dealing with our members over the phone with a strong clinical acumen in Heart Failure, COPD, and Diabetes conditions. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Hours: Monday – Friday: 8:00am to 4:30pm (flexible start and stop times), 1 Saturday/Sunday/Holiday (4-hour shift) per month

You’ll enjoy the flexibility to telecommute * from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:

  • Make outbound calls to assess members' current health status – heart failure, COPD, and diabetes
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management
  • Make referrals to outside sources
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews and connections to resources
  • This position will require active and unrestricted Nursing licensure in multiple US States. Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)

    Required Qualifications:

  • Current, unrestricted RN license in the state of Residence
  • Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses
  • 3+ years of experience as a Registered Nurse in a hospital, acute care or direct care setting
  • Intermediate / proficient level of experience with MS Office applications
  • Willing / able to obtain CCM within 30 months of employment if you do not already have it
  • Dedicated, distraction-free workspace in home for home office
  • Access to high-speed internet (DSL or Cable)
  • Ability to work 1 (4-hour shift) Saturday/Sunday/Holiday per month
  • Preferred Qualifications:

  • BSN
  • A background in managed care
  • Case management experience
  • Certified Case Manager (CCM)
  • Experience or exposure to discharge planning
  • Previous experience in a telephonic role
  • Home health experience
  • Ability to talk and type at the same time and have the ability to navigate a Windows based environment
  • Ability to handle multiple tasks and function in a high-volume environment

    Colorado, Connecticut or Nevada Residents Only: The salary range for Colorado residents is $53,300 to $95,100. The salary range for Connecticut / Nevada residents is $58,800 to $104,600.
