The primary role of the Patient Care Coordinator is to support the nursing team with scheduling patient visits for home-based, alternate care settings and ambulatory infusion suites were applicable, and creating/completing clinician visit logs confirming nursing visits for timely billing. Additional responsibilities include triage of patient phone calls, as well as tracking, faxing and timely follow up of physician orders.

The Patient Care Coordinator also performs other duties as needed based on experience level, including but not limited to assisting with intake tasks.

This position is full-time (40 hours/week) Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 9:00 am – 5:30 pm ( Monday-Thursday ) and 9:00 am – 6:00 pm (Friday). It may be necessary, given the business need, to work occasional overtime.

Primary Responsibilities:

  • Responsible for the coordination and scheduling of patient visits in the home and/or AIS, as applicable.
  • Assures timely consistent communication with company personnel and outside providers, in particular with patients, caregivers, and other agencies.
  • Assists with nurse training on Click scheduling and other applications, serving as a resource for questions and troubleshooting as experience deepens.
  • Tracks/documents and faxes nursing orders (POT/485’s) and supplemental physician orders for timely return with appropriate signatures.
  • Assists with triaging of patient phone calls for nursing.
  • Assists with the management of Click scheduling, timesheets, and activity recording for Optum Infusion nurses.
  • Assists with staffing patient visits to subcontracted home health agencies, including sending/retrieving all required documentation for patient management.
  • Confirms Clinician Visit Logs (CVLs) to meet Company nurse visit billing goals; includes review of nurse visit notes for accurate, consistent information during the confirmation process and facilitation of updates when needed to achieve timely nurse billing.
  • Verifies appropriate nursing visit authorization is obtained at Intake, and collaborates with DON when visit needs exceed authorization.
  • Monitors patient hospitalizations and/or events for completion of the required Performance Improvement Program (PIP) event reports by the reporter.
  • Generates reports through CPR+, Carelink 2.0, Nursing Dashboards and Click as requested.
  • Attends/assists in Branch/Regional nurse meetings.
  • Assists with ordering and restocking of nursing supplies as needed.
  • Verifies patient demographics during the visit scheduling process at start of care (SOC).
  • Complies with Company policy regarding the timely completion of required training programs (LearnSource and other programs as assigned); completion of common review-self evaluations; and entry of daily time in Global Self-Service (GSS) system.

    Required Qualifications:

  • High School Diploma / GED (or higher) or equivalent
  • Experience with Microsoft office applications – Microsoft Word, Excel, PowerPoint, Outlook (ability to create, edit, save and send documents, spreadsheets, presentations and correscpondence)
  • Preferred Qualification:

  • Experience in Medical Assistance
  • Soft Skill:

  • Excellent interpersonal, communication and organizational skills /multitasking.
  • Able to work with a cross-functional team of professionals including dietitians, pharmacists and pharmacy technicians, nurses, case managers, discharge planners, physicians, outside nursing agencies, patients, families and caregivers.
