Hi, we're Oscar. We’re hiring a Nurse Case Manager to join our Member Care and Services Department.

About the role:

We are a small team of nurses and social workers who have the privilege of meeting members where they are with the goal of helping members with complex medical needs improve their health.

You are an experienced practitioner who has the desire to work both collaboratively and independently with both internal and external resources. You have excellent communication and clinical skills and are comfortable thinking “outside the box” to help solve your members’ challenges.

You will report into the Case Management Lead. This is a remote role, you will work remotely in one of the following states: Arizona, California, Connecticut, Florida, Georgia, Illinois, Maryland, Massachusetts, Michigan, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Tennessee, Texas, Utah, Virginia, or Washington. Note, this list of states is subject to change.

Responsibilities:

  • Work with non-clinical teammates in assisting your members navigate the healthcare system
  • Communicate with your members via phone or secure messaging using your clinical expertise in an unscripted way to provide education on health conditions, new medications, and procedures
  • Coach your non-clinical teammates on how they can help you add more clinical value to all our member interactions
  • Assist in the coordination of care across a variety of settings (outpatient, ER, home care) while maintaining strict confidentiality and the highest level of professionalism
  • Actively reach out to members undergoing difficult health challenges, develop care plans with them, and help guide them through the healthcare system to improve the overall experience
  • Requirements:

  • 2+ years of clinical experience
  • Active, unrestricted AZ license (RN) and ability to obtain additional licensure as needed
  • Bonus points:

  • Working knowledge of Milliman Care Guidelines 
  • CCM certification
  • BSN
