About the role:
We are a small team of nurses and social workers who have the privilege of meeting members where they are with the goal of helping members with complex medical needs improve their health.
You are an experienced practitioner who has the desire to work both collaboratively and independently with both internal and external resources. You have excellent communication and clinical skills and are comfortable thinking “outside the box” to help solve your members’ challenges.
You will report into the Case Management Lead. This is a remote role, you will work remotely in one of the following states: Arizona, California, Connecticut, Florida, Georgia, Illinois, Maryland, Massachusetts, Michigan, New Jersey, New York, North Carolina, Ohio, Pennsylvania, Tennessee, Texas, Utah, Virginia, or Washington.
