Position Purpose:
Coordinate services between Foster Care staff, primary care physician (PCP) specialist, health plan case management team, and other medical and non medical providers as necessary to meet the complete medical socio-economic needs of member Perform member outreach, evaluation, and assessment Collaborate with case management team to make recommendations and referrals on necessary care plans Identify, recommend, and provide training for patients and foster child guardians. Education/Experience:
Bachelor’s degree in Social Work, Nursing, Health, Behavioral Science or equivalent experience. 2+ years of experience working within a social service agency and/or advocacy environment.
** Once work from home orders have been lifted, position will be required to work in local DCFS offices Springfield, IL office with the opportunity remote options and hybrid work schedules.**
License/Certification:
Valid driver’s license. RN (Registered Nurse), LCSW (Licensed Certified Social Worker), LPN (Licensed Practical Nurse), Occupational Therapy (OT), Physical Therapy (PT), or LVN (Licensed Vocational Nurse) preferred.
